Please ensure that someone is contactable at all times during your dog’s appointment in the event of an emergency or unexpected issue. Appointment Reminders and communication will be via phone/text.

Medical & Behavioural Information

I will be able to tell on your dogs coat work that has been done prior. We will discuss the process so you full understand the work I will do each time on your dog. SG will always put the comfort of your dog first. By signing this form you agree I 'She Grooms' will complete the groom to best of my ability at this time.
Sometimes for the dogs comfort, finishing legs or ear tips are finished with scissors is required. By writing 'I understand' with you initials, you acknowledge we have discussed and you are happy with my professional judgement for your dog. No clipping will ever occur unless it is part of the breed standard for Pet Grooming. SG will always be transparent with you.
Please write 'I understand' and initial.
Please give full disclosure, such as body areas your dog is protective over and what level of aggression.
Please note, I am not a groomer who specialises in behavioural issues and reserve the right to either refuse to groom your dog or halt the process at any time, if I feel things are getting out of control and your dog is getting overly distressed.

Additional Information

Price: $150.00
This fee will be deducted from your groom. 48 hours notice is required for any cancellations, this is because it is hard for me to fill your spot at short notice.
Dogs with heavier coats will need more frequent visits, this is for comfort and safety. (i.e Cocker Spaniels, Wire Fox Terriers, Welsh Terriers, Scottish Terriers)
